A lot of it depends on the judge and where you live.

In my state, it's very difficult to lose custody. In cases of drug use/alcohol abuse there is usually programs offered to the parent first. In high conflict cases they involve a GAL. With abuse allegations, they will order family counseling and supervised visits. One parent losing custody is almost always last resort after every other avenue has been exhausted.
